Being discharged from hospital
Your discharge, guidance on further care and possible check-up are discussed with the personnel. You are also given contact details in case of any problems. If you doubt your ability to cope at home, do tell the personnel well in advance. Before leaving the hospital, tell the personnel which certificates and documents you shall need.
Discuss transport with the personnel. If your condition does not allow you to use public transport, you will be given respective certificate. If you are transferred to another hospital or health care centre, the hospital covers travel expenses.
